    Hello everyone,

    I got a patient with premature ejaculation. He had this problem since his first sexual relation when he was around 15. It is not why he is coming to the clinic, but it is something that bothers him. When asking more about it, there is of course a strong emotional component, being each time afraid of not being able to have a “normal” intimate moment. He also describes an “absence of movement” in the pelvic region, like pressure being built and not moving, something only to be released by ejaculation.
    I wonder from the Classical point of view which channel system would be the most appropriate to treat this condition ?
    I read that if we look from the Sinews point of view, the SP brings blood to maintain erection, that KI manage the movement of fluids and LV deals with the release. So I guess the LV would be the more appropriate ?
    With the emotional, I also guess the Luo would also be interesting to consider ?

    Hi Antoine,

    I would be curious to know what his pulses say? Are there any sinew channel pulses? Traditionally, I too would have gone for Liver, but in this case, I would also be curious to know about his overall condition, like digestion, sleep and the rest and then pick the channel that might help with most of his patterns. Liver is a good start, but my guess is that it might require more than just a sinew treatment, so yes the Luo would be applicable, but also you can combine liver with spleen if it fits the overall diagnosis. Perhaps even the PC sinew channel given that this is a chronic condition and would help with the emotional side of the condition, I think!?

    Hello Armin,

    From what I can gather, there is activity at the GB Sinew, the KI pulse is a bit to floating and tight. About the overall condition, the sleep is ok but he had digestive issues since a young age mainly with smelling stools and smelling winds that can be annoying for him. Otherwise he is quite healthy, has physical activities few times a week. Not specific relational issues otherwise.
    I’ll consider the PC next time as well.

    Hi Antoine,

    Given the digestive issues, I would probably consider both liver and spleen sinew channel. The smelly stool and gas would likely indicate some damp-heat in the lower burner, or some kind of a blockage in the lower burner/Dai Mai. Since this has been going on for a long time and since his second Jing cycle, you could also consider the 8extras: Dai, Ren, Yin Wei (transition to puberty that didn’t go so smoothly).

    I really like Armin’s ideas.
    Once you’ve cleared the lower burner, you might consider consolidating Dai Mai but also clearing fear from the Yuan level since it’s a Yuan level issue based in fear. The simplest of these treatments (very very powerful, though) is to bleed TH-5, KI-4 and then BL-58. Let us know how it goes, Antoine.

    Hello Ann and Armin,

    I apologise for my late answer, I have been pretty busy the past few months.
    Unfortunately I didn’t see the patient after writing this post. But going into the process of thinking about it is a really good exercise. Coming from the a Japanese tradition, I often in my mind tend to think right away at the Kidneys (primary channel) which is from my experience not often the best choice.
    Ann, I would be very interested if you could in your next chat with Cody talk a bit about the mechanism at play, from the Advanced Acupuncture point of view, for the healthy sexual life of both male and female. About the arousal, the fluid mechanisms etc. I find it difficult to find informations on the subject, and now with a bit of knowledge about the Sinew channels I wonder how all of these parts work together. Or maybe do you have a reading to recommend ?
